About the New Account Process

Thank you for choosing to open a checking account with Fidelity Bank. Please visit your nearest branch office to complete a checking account application. You will need to bring the following information for all account owners:

  • Name
  • Address
  • Social Security number (you must be a U.S. Citizen or permanent resident alien)
  • Date of birth (you must be at least 18 years old to apply)
  • Telephone (home, work, cell)
  • E-Mail address
  • Mother’s maiden name
  • One form of identification (driver’s license, Passport or government-issued ID), including the ID number, and expiration date

Identity Verification Necessary
To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask for your name, address, date of birth, Social Security number, mother’s maiden name and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents. Any information that we request will be safeguarded and used for the purpose intended.
